LPN / RN / MSW Care Manager

Eligibility:
Licensed RN , LPN, or Social Worker with an active, non- suspended Louisiana nursing or social work license (MINIMUM OF ONE YEAR EXPERIENCE IN LICENSED FIELD IS REQUIRED)
Home Health experience for LPN is a plus
Holds a current driver's license and has access to personal automobile and current auto insurance to perform in home visits
Access to home or mobile phone that can be used to contact clients
Access to personal laptop or computer AND internet access for documentation purposes
Responsibilities: Care Manager
Create an individualized care plan for assigned members based on the care manager's in home assessment
Visit assigned members weekly at their homes
Monitor for progress towards goals and adjust care plan accordingly
Assist members with acquiring needed community resources
Assess medications and teach members and/or caregivers on current, new, or changed medications
Provide documentation within 24 hours of visits or communications regarding members
